The Chronic Care Improvement Program integrates disease management, case management and electronic care in order to improve the quality of care for chronically ill clients enrolled in Missouri Medicaid. The program provides education and health management support through the first care management tool that works in conjunction with an Internet-based plan enabling all participants -- patients, providers and health coaches -- to work more effectively together using a collaborative medical record. The program severs Medicaid patients identified through a risk assessment and disease stratification model mainly specializing on patients who suffer from: Asthma, Cardiovascular disease, Chronic Obstructive Pulmonary disease (COPD), Diabetes, Gastro esophageal and Reflux disease. The focus of the program is to improve the quality of care, decrease complications associated with chronic illness and reduce costs associated with delivering care to this population.
