Los Estados miembros de la UE se enfrenta cada vez más desafíos comunes con la creciente carga de enfermedades crónicas en la Unión Europea. Las enfermedades crónicas representan el 86% de la mortalidad en la Región Europa de la OMS y tener importantes implicaciones económicas. Las enfermedades crónicas requieren "la administración continua durante un período de años o diez años y cubren una amplia gama de problemas de salud" (OMS, 2002).
